What is GAPP?
The Georgia Pediatric Program (GAPP), managed by the Georgia Department of Community Health (DCH), offers the opportunity of at-home nursing care and personal care to medically fragile children under the age of 21. Children of families eligible for Medicaid can apply for GAPP at any time and must meet the same level of care necessary for admission to a hospital or skilled nursing facility.
